How lesson inquiry information is used.

Information collected

The lesson inquiry form collects a parent or guardian name, email address, phone number, student name and grade, instrument, experience level, preferred lesson times, and optional goals or comments.

Why it is collected

This information is used to respond to lesson inquiries, discuss availability, and determine whether lesson placement may be appropriate. Please do not submit sensitive student records, medical information, or school records.

How information is delivered

When the inquiry form is available, it sends submitted information to a Google Apps Script web app, which records it in the studio’s Google Sheet for follow-up.

Sharing and deletion

The studio does not sell inquiry information. Google processes form-submission data according to its own terms and privacy practices. To request deletion of an inquiry, email suzulose@gmail.com.
